Your annual review – Other Conditions

Atrial Fibrillation, CHD, Dementia, Heart Failure, Hypertension, Mental Health, Stroke/TIA

As you have one of the above conditions and are due for an annual review, we would like to invite you in to the surgery for this.

Please call the reception team on 01392 432761 to book a 30 minute appointment with one of our nurses, Sarah or Ruth. Please bring a urine sample with you to the appointment – this should be the first specimen of the morning.

We are working in a new way to make sure we carry on doing important patient reviews whilst keeping staff and patients as safe as possible whilst Covid-19 is active. In order to keep your surgery as safe as it can be, we are keeping any patients with Covid-19 symptoms away from the main surgery, cleaning the consultations rooms regularly and wearing PPE when the rate of infection in the community is high. There are antibacterial wipes and hand gels available in our waiting rooms for you to use freely.

We are running a new system which invites all patients in for their annual check-up during their birthday month. This means that you might be called earlier or later than you were last year.

We hope to see you soon in the surgery but if you are unable to come in, please let a member of the team know.
