We would like to invite you in for your diabetic annual review. It is important you attend, particularly if we haven’t seen you in the last 12 months for a review.
Please call the reception team on 01392 432761 to book a 30 minute appointment with our Nurse, Kerrie, who is here on Tuesdays and Thursdays Please make sure you bring a urine sample to this appointment- this should be the first specimen of the morning. If you need further review by your GP following the appointment, a member of our team will contact you to arrange this.
We run a system which invites all patients in for their annual check-up during their birthday month. This means that you might be called earlier or later than you were last year.
We hope to see you soon in the surgery but if you are unable to come in, please let a member of the team know.
