We would like to invite you in for your annual asthma/COPD review. It is important you attend, particularly if we haven’t seen you in the last 12 months for a review.
Please call the reception team on 01392 432761 to book a 30 min appointment with our Practice Nurse, Sarah.
Please bring your inhalers with you to the appointment and make sure you take them as normal on the day.
If you have a mobile or email address, we may be able to send you an asthma questionnaire to complete and you will only need to attend if your control is poor, or you have other conditions which need a face to face review.
We run a system which invites all patients in for their annual check-up during their birthday month. This means that you might be called earlier or later than you were last year.
We hope to see you soon in the surgery but if you are unable to come in, please let a member of the team know.
